Who is this service for?
- People who have received a first prescription for a new medicine for eligible long-term conditions
- Common eligible areas include asthma, COPD and other chronic lung conditions
- It also includes conditions such as type 2 diabetes, high blood pressure, high cholesterol, osteoporosis, gout and glaucoma
- Cardiovascular conditions may also be included, such as heart failure, coronary heart disease and atrial fibrillation
- The service may also cover medicines that affect blood clotting, including anticoagulants and antiplatelets
